So sorry to hear that  :(, it's a pity you can't name and shame. (Or can you?)

David Asquith

Really sorry to hear this Mike.  The pain of your loss was bad enough without knowing it could have been avoided.  Our thoughts are with you.

Mike, that is horrifying. I don't really have words for this moment but please be aware that my thoughts are with you and your son right now.

With all sympathy, Allan.....

Awful news to read. My sympathies with you all.
Any opinion expressed above is mine and mine alone. Unless you happen to agree. Then they're your's too.

I'm so sorry to hear that. To lose your loved one is bad enough but through human error is dreadful. There must be some recourse to justice for something like this. My thoughts are with you.
You know you're getting older when your mind makes commitments your body can't meet.

That would be very unwise on a public forum - libel plain and simple.

With all due respect, it's only libel if it is not true.
The years have been good to me, it was the weekends that did the damage.

Sprintex

And that is for the inquest to decide, until then any statements could be considered libellous.

The report is a comprehensive  investigation from the chief medical officer

My sympathies are with you and your son, Mike - I can only imagine the heartbreak that you must be going through right now.  :(
Just one Pendolino, give it to me, a beautiful train, from Italy
