You can get laminators pretty cheap and they usually do photo ID sized lamination wallets.. (goes to check amazon.. )

like this, which does up to A4

http://www.amazon.co.uk/Monolith-A4-Laminator-OL-289/dp/B00KGSQ190/ref=sr_1_2?s=officeproduct&ie=UTF8&qid=1455817566&sr=1-2&keywords=Laminator

and suitably sized pouches like this...

http://www.amazon.co.uk/50-Laminating-Pouches-Credit-Card/dp/B0042FQLL6/ref=sr_1_11?s=officeproduct&ie=UTF8&qid=1455817669&sr=1-11&keywords=photo+ID+pouches

Easy way would be to pre-print some credit card sized ID cards with blank name/sig lines on your computer with a space to insert a passport sized photo. When you get a new member, scribble their details and signature on a card, stick it together with the photo into a pouch and laminate.

I also noticed in the search though that you can get 'self laminating' ID card pouches. Essentially a plastic pouch that just sticks together once you've put the card inside. So that may be a cheaper option.

DELETED

I've had some experience in setting up company ID cards and those plastic access cards used for access over the past few years -the ones with radio tags or magentic strips.  Practically speaking it's quite easy to set something very basic up at little cost.

. Make up a standard form in MS word, and parental consent (signature) at the bottom for under 18's
. Make up a standard numbering for your cards
. Create a main folder which you can use as a database, create a new sub-folder inside for each person, inside that folder is the scanned consent form (might as well do one for everyone regardless of age) andf a small photo.  Name each of the files starting with the card number and name
. You may want to create an excel spreadsheet, or just a basic list in word of the cards you've issued, the date of issue, expiry (if req.) and the person's name / details
. For the cards I would say by far the easiest is to use Powerpoint.  Measure the size of a business card and draw a box with a black border (maybe make it a couple of mm smaller on each edge).  Make up a standard set of info you want to have on each card and space it out how you want it.  Choose a photo size and create another box with a black border and place it where you want.  Now step back and select everything you have drawn, right click and select "group".  When you want to create a new card treat this as the "background", draw a little text box dor each little detail you want, drag a photo into Powerpoint from file explorer and press "shift" as you drag a corner to the right size of your picture box (or just re-size the picture remembering to keep aspect ratio OK)
. Print out your card -I would have the card set at the top left so you can just use a half sheet of A4 -plain paper or thin card might do (you can always copy and paste a whole heap to fil up afull A4 page in powerpoint)
. A laminator is (or was) about £17 from tescos, cut out your cards and space them out in the pouch so there's a clear border round each one.  Laminate them and cut-out so there's a thin sealed border round each one -if you don't leave a seal round each one then moisure will get in and wreck the card
. Lastly, make sure you use a shredder for any info on paper collected which you don't need.  Cheap £10 one would do if you don't have access to one anyway

...Tht's what I'd do anyway.  Very quick and simple.  I don't know about data protection laws other than what I've experienced myself.  I would strongly recommend before you start collecting any personal information that you draw-up a method statement contining a statement of confidentiality saying what the info will be used for and guaranteeing you will not pass on the info.  Names, addresses, DOB's, photo's etc etc are very sensitive info these days (I would guess very much so for children) so make sure you've covered yourself FIRST as it's amazingly common to be challenged over anything outwardly innocent these days.

If you wanted any help with std formats I could put something together over this weekend for you if you drop me a PM with an E-Mail address.

Hope this helps,

Rich
